Our goal is to provide you with quality, individualized mental health care. Your mental health is as important to us as it is to you, therefore, we take your care seriously. “No-shows” and late cancellations may prevent those individuals who need access to care from being seen. Additionally, providing care is not sustainable unless bills are paid in a timely manner.
Appointment Cancellations:
Please be courteous and call our office promptly if you are unable to come for your appointment. We require that you call at least 24 hours in advance. Appointments are in high demand and your early cancellation will give another person the chance to have access to timely mental health care. There will be no charge for properly cancelled appointments.
No Show/Late Cancellation Policy:
A “no-show” is when someone misses their appointment without cancelling. If you cancel within 24 hours of your appointment, unless there is a serious unforeseen circumstance, it will be considered a late cancellation and noted in your medical record as a “no-show”. The following fees will be charged in these circumstances:
First “no show”/late cancellation in a 6 month period: No charge
Second “no show”/late cancellation: $50.00 fee will be billed to your account and payable at the next appointment before you will be seen.
Third “no show”/late cancellation: $100.00 fee will be billed to your account and payable prior to your next appointment before you will be seen.
Please note insurance does not cover the above fees, so you are solely responsible. If you have three “no-shows” or late cancellations in a 6 month period, you may be subject to termination from Catalyst Psychiatry.
Refills and Missed Appointments:
Patients who are on controlled substances (stimulant, benzodiazepine) that no show or late cancel for their scheduled appointment will not receive a refill until you are seen again by your psychiatrist. Other medication refills will be at the discretion of your psychiatrist.
Telehealth Appointments:
If you are seen via telehealth, you agree to have a credit/debit card on file. By signing below, you agree to Catalyst Psychiatry billing your co-pay/co-insurance amount on the day of service to the card on file.
Cash Pay Patients:
You agree to have a credit/debit card on file. By signing below, you agree to Catalyst Psychiatry billing your visit fees in full to the card on file, minus a 25% cash discount.
Outstanding Account Balances:
Outstanding balances of greater than $100 for more than 45 days must be paid in full or a payment plan set up and adhered to via keeping a credit/debit card on file before any new appointments will be made or refills for medications given. By signing below you agree to Catalyst Psychiatry billing your payment(s) to the card on file.
